1．膠原病・血管炎の急性期治療：state-of-the-arts 2018—寛解導入療法の温故知新

Current induction therapy for the treatment of patients with rheumatic diseases

ステロイドの台頭により，膠原病・血管炎の治療が可能となったが，ステロイド長期使用に伴う有害事象と戦うことにつながった。この数十年の間にステロイドのみのシンプルな寛解導入から，免疫抑制剤もしくは生物学的製剤を併用した早期の寛解導入へと変化し，ステロイドによる有害事象を減らす治療戦略が主流となってきた。また，免疫グロブリン大量療法や血漿交換療法などの古典的な治療も，一部の血管炎・膠原病では今なお寛解導入療法として使用されている。ICUなどの急性期治療でも行われる可能性のある，新旧含めたリウマチ膠原病の寛解導入療法を概観する。

●寛解導入療法に関する大規模な臨床試験が次々と報告され，治療初期から生物学的製剤や免疫抑制剤などの“steroid-sparing agents”を使用することが主流になりつつある。

●上記薬物の台頭に伴い，免疫グロブリン大量療法や血漿交換療法の出番が少なくなってきているが，血管炎・膠原病の一部では高い有効性を示すものがある。

Glucocorticoid therapy enables the treatment of rheumatic diseases, while the long-term use of glucocorticoids has an unfavorable effect. Over recent decades, combination therapy with immunosuppressants or biologics is increasingly being used in clinical practice to reduce the toxicity associated with glucocorticoid therapy instead of glucocorticoid monotherapy. Intravenous immunoglobulin therapy and plasmapheresis can be a useful option for the treatment of some rheumatic diseases and vasculitides. In this paper, we describe classical and state-of-the-art induction therapy for the treatment of patients with rheumatic diseases which are used in critical care.
