Autoimmune diseases and systemic vasculitis rarely present with acute cardiogenic or obstructive shock. Therefore, intensivists may fail to consider that these diseases are causing circulatory shock. However, some patients with these diseases require admission to the ICU due to circulatory shock, which may be life-threatening if not treated with appropriate immunosuppressant therapy. This review focuses on clues for diagnosing rheumatologic diseases and severe cardiac conditions including myocarditis due to systemic lupus erythematosus and eosinophilic granulomatosis with polyangiitis, coronary arteritis from polyarteritis nodosa, and pulmonary arteritis from Takayasu arteritis. Another rheumatologic issue potentially troublesome for intensivists is the management of thrombotic events associated with antiphospholipid syndrome. The optimal antithrombotic therapy has been a topic of debate. In the last section, the selection of antithrombotic therapy is discussed with reference to the latest evidence including what is currently unknown or uncertain.
