4．膠原病・血管炎による急性循環不全：破れる・詰まる—致死的病態に潜む膠原病の疑い方・見つけ方

Acute circulatory failure due to autoimmune diseases or systemic vasculitis

膠原病のpresentationとして急性循環不全は珍しく，原疾患の鑑別診断リストに膠原病は挙がりにくい。一方，一部の膠原病では確かにICU入室が必要なほどの循環不全をきたし，免疫抑制剤による介入をしなければそのまま悪化し，亡くなり得る。全身状態が不安定で診断に多くの時間を割けないICUで循環不全の陰に潜む膠原病を見落とさず診断し治療につなげるにはどうしたらよいのか。具体例を通して診断から治療への流れを説明する。心筋炎に関しては全身性エリテマトーデス（SLE），好酸球性多発血管炎性肉芽腫症（EGPA）を冠動脈，腸間膜動脈など中動脈の破綻に関しては結節性多発動脈炎（PN）を，肺動脈の病変からの循環破綻に関しては高安動脈炎を取り上げる。最後に治療に関する悩みがつきない抗リン脂質抗体症候群を扱う。

Main points

●改善に乏しい心筋炎をみたら他の臓器障害とあわせて全身性エリテマトーデス（SLE）や好酸球性多発血管炎性肉芽腫症（EGPA）など膠原病として説明がつくかどうか一考する。

●冠動脈，腎動脈，腸間膜動脈など中動脈の閉塞・破綻に不自然な炎症を伴っていたら結節性多発動脈炎（PN）を疑い生検可能な障害臓器の特徴的画像所見を検索する。

●肺血栓塞栓症のようなpresentationで来院した患者の肺動脈狭窄をみたら高安動脈炎の可能性を考える。

●若年・unprovoked・再発の血栓塞栓症例では抗リン脂質抗体チェックを忘れずに行う。抗リン脂質抗体症候群では診断と同時にリスク分類を行い抗血栓薬開始後も再発に注意する。

Autoimmune diseases and systemic vasculitis rarely present with acute cardiogenic or obstructive shock. Therefore, intensivists may fail to consider that these diseases are causing circulatory shock. However, some patients with these diseases require admission to the ICU due to circulatory shock, which may be life-threatening if not treated with appropriate immunosuppressant therapy. This review focuses on clues for diagnosing rheumatologic diseases and severe cardiac conditions including myocarditis due to systemic lupus erythematosus and eosinophilic granulomatosis with polyangiitis, coronary arteritis from polyarteritis nodosa, and pulmonary arteritis from Takayasu arteritis. Another rheumatologic issue potentially troublesome for intensivists is the management of thrombotic events associated with antiphospholipid syndrome. The optimal antithrombotic therapy has been a topic of debate. In the last section, the selection of antithrombotic therapy is discussed with reference to the latest evidence including what is currently unknown or uncertain.
