（V−18）　50歳男性が左眼視力低下で受診した。2日前に鍬で土壌を耕していて左眼に異物感を自覚している。矯正視力は右1.5，左光覚弁，眼圧は左41mmHgであった。左眼に外傷性白内障と前房蓄膿があった。Bモードエコーとコンピュータ断層撮影で眼内に異物があり，同日，硝子体手術を行った。術中の所見として，内境界膜下に多量の滲出物があり，これの除去に内境界膜剥離が必要であった。眼内レーザーと拡張性ガスによるタンポナーデを行って手術を終わった。手術から4か月の現在，左眼矯正視力は0.08で，炎症所見はない。眼内異物が原因で受傷後早期に発症した眼内炎に対して，速やかな硝子体手術が有効であることを示す症例である。

A 50-year-old male presented with blurring of vision in his left eye. He had a foreign body sensation in the left eye while tilling the ground with a hoe. His left visual acuity was light perception. The intraocular pressure was 41 mmHg. The left eye showed traumatic cataract and hypopyon. B -mode echography and computerized tomography showed an intraocular foreign body which was removed by vitrectomy the same bay. As intraoperative finding, massive exudate was present beneath the internal limiting membrane (ILM). We removed the exudate after artificial detachment of ILM and added endolaser photocoagulation and gas tamponade. His visual acuity is 0.08 4 months after surgery without signs of intraocular inflammation. This case illustrates that prompt vitrectomy is indicated for endophthalmitis which manifest soon after penetrating ocular injury.
