（P3-1-7）　眼科受診がHlV抗体陽性の発見の契機になった桐沢型ぶどう膜炎の1例を経験した。症例は41歳の男性で，右眼の霧視，充血で発症し，眼底に網膜壊死病変が認められた。初診時の血液検査でHIV抗体陽性であったため，一時はアシクロビルとガンシクロビルの併用療法を行った。診断確定には前房水のPCR法が有用であった。網膜壊死部は瘢痕化し硝子体混濁は軽度残存したが，網膜剥離は発生しなかった。矯正視力（1.2）を保っている。網膜壊死がみられる症例ではHIV抗体検査を早期に行い．診断の手掛かりにする必要がある。HlV抗体陽性例ではHIV治療を平行して行うことが，原疾患の予後を向上させる手助けになる。

A 41-year-old male presented with hyperemia and blurring of vision in his right eye since 2 weeks before. The affected eye showed signs of iridocyclitis and acute retinal necrosis. Laboratory studies showed human immunodeficiency virus (HIV) antibody in the blood. He was treated by systemic acyclovir and ganciclovir. Aspirated anterior aqueous processed by polymerase chain reaction led to the definitive diagnosis of HIV infection. During the course of treatment, no retinal detachment developed in spite of persistent vitreous opacity. The visual acuity improved from the initial 0.3 to 1.2. This case illustrates the usefulness of HIV studies in the diagnosis and treatment of acute retinal necrosis.
