A 76-year-old male underwent uneventful self-sealing cataract surgery and intraocular lens implantation in his right eye. Dendritic keratitis developed on day 18 of surgery. It improved following topical acyclovir and ofloxacin. Necrosis of the scleral wound with endophthalmitis developed on day 53. Ofloxacin-resistant coagulase-negative staphylococci (CNS) were isolated from the necrotic scleral flap. Intensive treatment with systemic and topical antibiotics led to clinical cure. This case illustrates the importance of pathogenicity of CNS, which are present in the normal conjunctival sac, following cataract surgery.
