Abstract. Purpose:To report a case of Tolosa-Hunt syndrome associated with temporary occlusion of retinal and choroidal artery. Case:A 64-year-old male sought advice for malaise and vomiting with left orbital and frontal pain. He had had a similar attack 3 days before with diplopia. Findings:Visual acuity could not be tested for poor systemic status. The left eye showed restriction in upward and lateral gaze. Immediately thereafter, the patient noticed acutely impaired vision in the left eye. Corrected visual acuity was 0.9 right and no light perception left. The left fundus showed narrowing of retinal vessels. Fluorescein angiography showed delayed dye inflow into retinal and choroidal vessels. Magnetic resonance angiography(MRA)showed irregular internal carotid artery in cavernous sinus with apparently occluded ophthalmic artery. The patient started treatment with pulsed corticosteroid under the diagnosis of Tolosa-Hunt syndrome. Left visual acuity improved to 0.3 the following day. There has been no recurrence for 7 months until present. Conclusion:This case illustrates that temporay occlusion of ophthalmic artery may be associated with Tolosa-Hunt syndrome.
