A 57-year-old female presented with upper visual field defect in her left eye since 6 days before. Her corrected visual acuity was 1.0 right and 0.05 left. Funduscopy showed pale swelling of the left optic disc with indistinct disc margin. Hemorrhagic patches and arterial narrowing were present inferior to the disc. These and fluorescein angiographic findings led to the diagnosis of anterior ischemic optic neuropathy (AION) of the left eye. Optical coherence tomography (OCT) showed increased thickness of retinal nerve fiber layer inferior to the disc and reduced thickness superiorly on day 9 of the disease. The retinal nerve fiber layer showed diffuse thinning on day 68. Although the thinning showed a slight tendency to recover thereafter, the final visual acuity was 0.02. This case illustrates that the retinal fiber layer may be affected during the early phase of AION.
