　57歳女性に左眼の上方視野欠損が突発し，その6日後に受診した。矯正視力は右1.0，左0.05であった。左眼の視神経乳頭は境界不鮮明で蒼白浮腫状であり，その下方の網膜に出血斑と動脈の狭細化があった。蛍光造影所見などから，左眼の前部虚血性視神経症と診断した。発症9日目の光干渉断層計による検索で，左眼の神経線維層厚が乳頭下方では増加し，上方では菲薄していた。発症68日目には，神経線維層厚が乳頭の全周で著しく菲薄化していた。その後，菲薄化はやや回復したが，最終視力は0.02であった。前部虚血性視神経症では，早期から神経線維層の異常が生じることを示す症例である。

A 57-year-old female presented with upper visual field defect in her left eye since 6 days before. Her corrected visual acuity was 1.0 right and 0.05 left. Funduscopy showed pale swelling of the left optic disc with indistinct disc margin. Hemorrhagic patches and arterial narrowing were present inferior to the disc. These and fluorescein angiographic findings led to the diagnosis of anterior ischemic optic neuropathy (AION) of the left eye. Optical coherence tomography (OCT) showed increased thickness of retinal nerve fiber layer inferior to the disc and reduced thickness superiorly on day 9 of the disease. The retinal nerve fiber layer showed diffuse thinning on day 68. Although the thinning showed a slight tendency to recover thereafter, the final visual acuity was 0.02. This case illustrates that the retinal fiber layer may be affected during the early phase of AION.
