A 41-year-old male presented with hyperemia and blurring of vision in his right eye since 2 weeks before. The affected eye showed signs of iridocyclitis and acute retinal necrosis. Laboratory studies showed human immunodeficiency virus (HIV) antibody in the blood. He was treated by systemic acyclovir and ganciclovir. Aspirated anterior aqueous processed by polymerase chain reaction led to the definitive diagnosis of HIV infection. During the course of treatment, no retinal detachment developed in spite of persistent vitreous opacity. The visual acuity improved from the initial 0.3 to 1.2. This case illustrates the usefulness of HIV studies in the diagnosis and treatment of acute retinal necrosis.
