屈折型多焦点眼内レンズの術後成績と適応

Postoperative results and indication of refractive multifocal intraocular lens

（B2-1-3）　直径2.1mmの中心遠用ゾーンを持つ屈折型多焦点眼内レンズの術後成績を検討した。対象39眼のうち，1.0以上の遠方矯正視力が得られた症例は92％，0.7以上の近方矯正視力が得られた症例は，近方矯正下92％であったのに対し，遠方矯正下33％であった。44％が遠近で眼鏡の使い分けを必要とした。59％が術後成績に満足したが，18％は不満であった。当レンズは，従来の屈折型多焦点眼内レンズと比較して遠方矯正下近方視力が低かったが，50歳未満の症例に関しては，全例が眼鏡を必要とせず，術後成績に満足していた。しかし，加齢により多焦点効果が低下する可能性も示され，長期的な観察が必要と考えられた。

We reviewed 39 eyes of 27 patients who received implantation of refractive multifocal intraocular lens (IOL) with a central distance zone of 2.1 mm in diameter. Distance visual acuity of 1.0 or better was obtained in 92% of eyes with correction. Near visual acuity of 0.7 or better was obtained in 92% of eyes with correction and in 33% with distance correction. Different pairs of spectacles were needed for near and distance vision in 44% of 27 patients. The postoperative results were judged as satisfactory by 59% and not satisfactory by 18% of patients. It appeared that near visual acuity with distance correction of the current IOL was inferior to other types of refractive IOL. All patients younger than 50 years of age did not need spectacles and were satisfied with the results. There were findings suggesting that the effects of multifocal IOL would be diminished along with aging.
