Abstract 文献概要

　コーバン屈折型多焦点眼内レンズ（ストルツ，米国）の臨床試験を42例53眼に行い，脱落症例5眼を除いた48眼についてその術後成績を検討した。光学径が異なる68—STUVとP359—TUVの2種類を使用し，平均観察期間は21.7か月と10.8か月であった。術後の遠方矯正視力が1.0以上であった症例の割合は，68—STUV 33眼中29眼で82％,P359—TUV 15眼中13眼で87％であった。遠方矯正時の近方視力が0.7以上であった症例の割合は，それぞれ88％と93％であった。全例で眼鏡不要あるいは1種類の眼鏡のみで遠方0.7以上，近方0.4以上の視力が得られた。患者の印象は「良かった」以上がいずれも100％であり，また，安全性に関して問題はなかった。有用性の評価は，全例で「有用」以上であった。特に小さいモデルであるP359—TUVは術後の乱視コントロールが容易であり，裸眼視力の良好な症例が多く，遠近とも眼鏡不要と考えられた症例の割合が高かった。以上から，当レンズは臨床上有用な多焦点眼内レンズであると考えられた。

We implanted Coburn refractive multifocal intraocular lens (IOL) in 53 eyes and evaluated the results in 48 eyes excluding 5 dropouts. Two models with different optic size were used: 68-STUV in 33 eyes and smaller P 359-TUV in 15 eyes. The former was followed up for 21.7 and the latter for 10.8 months. Corrected distance visual acuity (DVA) of 1.0 was obtained in 82% in the former and 87% in the latter. Near visual acuity (NVA) of 0.7 or better was obtained in 88% in the former and 93% in the latter. With or without distance correction, all the cases obtained DVA of 0.7 or better and NVA of 0.4 or better. The subjective impression of patients was better than 'good' and better than 'effective' in all. Adverse reactions were consistently absent. Postoperative astigmatism could be easily controlled, particularly in the latter type. It is assessed as clinically useful because of high incidence of good visual acuity for near and distance without correction.
