Abstract Purpose: To report background factors of cases who needed exchange of multifocal intraocular lens(IOL).

Cases and Method: This retrospective study was made on 24 eyes of 19 cases who received exchange of multifocal IOL in the past 6 years. The series comprised 7 males and 12 females. The age ranged from 49 to 75 years, average 63 years. Multifocal IOL had been inserted bilaterally in 14 cases, of whom IOL was exchanged in one eye only in 9 cases and in both eyes in 5 cases. Initial insertion of multifocal IOL had been performed in other insitutions in 22 eyes of 17 cases.

Findings: Multifocal IOL was diffractive type in 20 eyes and refractive in 4 eyes. The interval between initial insertion of multifocal IOL and exchange averaged 21.4 months. Ocular complications or prior surgery included LASIK 4 eyes, glaucoma in 4 eyes and epiretinal membrane in 2 eyes. Exchange of multifocal IOL was apparently due to waxy vision with discomfort in 16 eyes(67%).

Conclusion: Current multifocal IOLs result in dissatisfaction in some cases needing exchange of IOL. We advocate careful selection of indication and inform the patients about characteristics of multifocal IOL.
