要約　目的：眼内レンズ（IOL）固定困難症例に対するSulcus-Subcapsular fixation（SSF）の術後屈折誤差について検討する。

対象と方法：SSFは後囊破損時やチン小帯脆弱において，将来的にIOL偏位のリスクが高い場合に後囊温存時にもIOL支持部を毛様溝に置き光学部を水晶体囊の下に嵌め込む固定法である。SSFと囊内固定群（対照群）の術後3か月における屈折誤差について検討した。

結果：屈折誤差はSSF：−0.09±0.55D（n＝40），囊内固定：＋0.03±0.24D（n＝40）で有意差は認めなかった（p＝0.1794，マン・ホイットニーのU検定）。

結論：IOL固定困難症例に対するSSFは囊内固定と比較して有意な屈折誤差はなく，囊内固定と同度数のIOLを使用できる。

Abstract　Purpose：To evaluate the postoperative refractive error of the Sulcus-Subcapsular fixation（SSF）technique for intraocular lens（IOL）implantation（i.e., fixing the haptics to the ciliary sulcus and the optic to below the anterior or posterior capsule）in patients with difficult IOL fixation. This included patients experiencing posterior capsular rupture or zonular weakness.

Methods：We retrospectively evaluated refractive error at 3 months postoperatively in 40 eyes among 37 patients who had undergone the SSF technique and in 40 eyes among 32 patients who had undergone in-the-bag IOL fixation.

Results：The mean refractive difference was −0.09±0.55D with SSF and＋0.03±0.24D with in-the-bag IOL fixation. No significant difference was found between the two techniques（p＝0.1794, Mann-Whitney U Test）.

Conclusion：There was no significant difference in the refractive error of patients who underwent SSF compared to those who underwent in-the-bag IOL fixation. The same power can be used in both techniques.
