Abstract Purpose:We report a case of glaucoma in a patient with improved contrast sensitivity after multifocal intraocular lens exchange.

Observation:A 48-year-old woman with primary open-angle glaucoma(POAG)in both eyes and currently undergoing with eyedrops presented to the department treatment. She had undergone cataract surgery and trabeculotomy(LOT)in the right eye previously, which was performed by another doctor. A multifocal intraocular lens(IOL)was inserted, and YAG laser treatment was performed postoperatively. She visited our hospital for a second opinion owing to poor visual acuity after surgery. The pre-operative visual acuity of the right eye, as recorded by the previous doctor, was Vd=0.04(0.7×−6.50D=cyl−0.50D 80°), and the visual acuity at the first visit to our hospital was Vd=(0.15p×IOL)(0.6×−1.00D=cyl−2.00D 170°=IOL). The intraocular pressure in the right eye was measured with a Goldman tonometer and was found to be 18 mmHg. The mean deviation(MD)and foveal threshold on HFA24-2 for the right eye were −15.75 dB and 26 dB, respectively, and the contrast sensitivity was reduced. A three-piece multifocal IOL was used in extracapsular fixation, and a large posterior capsule incision was noted. It is standard practice to perform multifocal IOL exchange, and a monofocal IOL was replaced in the right eye. The corrected visual acuity after operation was 0.7, the contrast sensitivity had improved, and the discomfort had also improved relatively.

Conclusion:We report the case of a glaucoma patient with improved contrast sensitivity after multifocal IOL exchange. The use of multifocal IOLs in eyes with central visual field defects can significantly reduce visual function.
