身体障害者認定基準案によれば2級となる可能性のある5級の糖尿病網膜症症例

要約　目的：国際的基準であるFunctional Vision Score（FVS）によると，わが国ではロービジョン者が身体障害者手帳を取得できず，盲である人でも5級にしか認定されない。視覚障害の認定基準に関する検討会の2016年8月の認定基準改訂案では，求心性視野障害の偏心に対する対応の項目で「Ⅰ/4eによるイソプタの一部が10°を超えていても，15°以内でありかつ残余面積が中心10°視野と同等かそれ以下であれば，10°以内として判定し，4，3，2級判定へ進めるようにした。」とあった。この案では5級であったものが，今回の改訂案（2017年7月）でⅠ/4eの8主経線の合計が80°以内であれば2〜4級判定に進める案が示された。

症例：73歳，男性．糖尿病歴20年。10年前に他医で増殖期糖尿病網膜症と黄斑症の治療を受けている。今回他科入院中に眼科に紹介された。補助具を用いても読書不能で音声から情報を得るのみであり，障害物に気づかず，転倒することもしばしばであった。視力は右（0.3），左　光覚弁。網膜は黄斑，周辺とも菲薄化し，視野は右眼Ⅰ/4eが耳17°，耳上8°，上7°，鼻上7°，鼻5°，鼻下5°，下7°，耳下13°と8主経線合わせて69°で，80°以内であった。左眼は測定不能であった。現行の規定および2016年の改訂案では5級であったが，2017年の改訂案によると，Ⅰ/2eの加重平均が1.5°となり，2級と認定されることが予測された。一方，Functional Field Scoreは50，Functional Acuity Scoreは59，FVSは30となり，AMA分類では3b極度視覚喪失（盲）と同様の結果となった。

結論：今回の改訂案により，実際の視野障害に見合った等級判定がされる可能性が示された。

Abstract　Purpose：According to the current Functional Vision Score（FVS）, low-vision persons are to be classified as grade 5 and not as blind. A new proposal for revision was made to the effect that low-vision persons may be classified as grade 2, 3 or 4 when the isopter for Ⅰ/4e is within 15 degrees and the area of visual field is less than 10 degrees.

Case：A 73-year-old male had diabetes since 20 years before, and was treated for diabetic retinopathy and maculopathy 10 years before. Visual acuity was 0.3 right and light perception left. Visual field for Ⅰ/4e in the right eye was less than 80 degrees when the isopters for Ⅰ/4e along the 8 meridians are added together. Visual field could not be tested in the left eye. According to the standard for 2016, he was to be assigned to grade 5. After the proposed revision, he will be assigned to grade 2. Classification of American Medical Association shows that he is to be regarded as profound visual loss, or blind.

Conclusion：The proposed revision on visual field will mean that more patients will be assigned as low vision or blind.
