眼底所見の経過が追えた眼内悪性リンパ腫の1例

Clinical course of fundus lesions in a case of intraocular malignant lymphoma

　75歳女性が1か月前からの左眼視力低下で受診した。矯正視力は右1.0，左0.3であった。左眼眼底の耳側に大小の黄白色滲出斑と腫瘤性病変があり，広範な滲出性網膜剝離，網膜出血，びまん性硝子体混濁があった。右眼と全身には異常がなかった。1週後に前房に細胞が播種状に出現し，豚脂状角膜後面沈着物が生じた。初診から6週後に眼底腫瘤が拡大する一方，滲出斑と網膜炎様の所見は軽快した。さらに2週後，硝子体混濁が進行し，左眼視力は手動弁に低下した。硝子体の細胞診で悪性リンパ腫の診断が確定した。局所放射線照射で眼底腫瘍は瘢痕化し，0.2の視力を回復した。以後11か月後の現在まで経過は良好である。眼内の悪性リンパ腫はぶどう膜炎に類似した所見を呈することが多いが，進行性で広範な腫瘤形成を中軸病変とした一例である。

　A 75-year-old female presented with decreased vision in her left eye since one month before. Her corre-cted visual acuity was 1.0 right and 0.3 left. Her left fundus showed exudates of various sizes and tumorous lesion in the temporal sector in addition to extensive exudative retinal detachment,retinal hemorrhages,and vitreous opacity. Cell dissemination developed in the anterior chamber one week later. Six weeks after her initial visit,the tumorous lesion enlarged further followed by remission of exudates and inflammatory features. Vitreous opacity increased 2 weeks later and the visual acuity decreased to hand motion. Vitreous biopsy led to the diagnosis of malignant lymphoma. Focal radiation induced scarring of fundus tumor and improvement of visual acuity to 0.2. This case illustrates that tumorous lesion may be the outstanding feature in intraocular malignant lymphoma.
