濃度と分子量が異なる3種類の粘弾性物質を用いて59眼に白内障手術を行い,術後眼圧上昇を比較した。粘弾性物質は高濃度高分子群19眼(ヒーロンV(R)),高分子群15眼(ヒーロン(R)),中分子群25眼(オペリード(R))である。手術には超音波水晶体乳化吸引術と眼内レンズ挿入術を用い,粘弾性物質はbehind the lens法で吸引除去した。眼圧は術前24時間,術後5,24,48時間と1週間に測定した。平均眼圧は各群とも術後5時間で有意に上昇し,術後48時間で有意に低下した。眼圧変化値と変化率は各群間に有意差がなかった。白内障術後早期の眼圧上昇に,粘弾性物質の濃度と分子量は大きく影響しないと結論される。
We performed cataract surgery on 59 eyes using either of three viscomaterials. Nineteen eyes were operated with Healon V(R)that has high density and high molecular weight. Fifteen eyes were operated with Healon(R)that has high moleular weight. Twenty-five eyes were operated with Opelead(R)that has medium molecular weight. All the eyes received phacoemulsification-aspiration with intraocular lens implantation. The viscomaterial was removed by behind-the-lens method. Intraocular pressure(IOP)was measured 24 hours before surgery,5,24,48 hours and one week after surgery. The average IOP rose significantly 5 hours after surgery and decreased significantly 48 hours after surgery. There were no differences in the rate or amount of IOP changes among the three groups. The findings show that IOP during the early period after cataract surgery is not influenced by viscomaterials of different density and molecular weight.
