眼内悪性リンパ腫の2例

Two cases of intraocular malignant lymphoma

要約　目的：眼内悪性リンパ腫2症例の報告。症例：症例はいずれも女性で，年齢は73歳と69歳である。主訴はいずれも両眼の視力障害と霧視で，初診時の矯正視力は，0.2と光覚弁，0.01と0.4であった。前房と硝子体混濁が4眼すべてにあり，そのうちの2眼には広範囲の網脈絡膜萎縮があった。1例では硝子体手術による生検で，クラスVの異型リンパ球があり，悪性リンパ腫の診断が確定した。他の1例では副腎皮質ステロイド点眼が無効であった。硝子体生検では診断が確定できなかったが，硝子体中のサイトカイン検索でIL-10が高値であり，IL-10/IL-6比率が高く，悪性リンパ腫が強く疑われた。2例とも放射線照射を行った。結果：それぞれ9か月と13か月の経過観察で，再発または中枢神経系への転移はなく，最終視力は0.3と手動弁，1.2と1.2である。結論：眼内悪性リンパ腫に対する副腎皮質ステロイド点眼は硝子体細胞診の陽性率を下げる危険がある。硝子体中のIL-10とIL-6の測定が補助診断として有用である。

Abstract.　Purpose：To report two cases of intraocular malignant lymphoma. Cases：Both were females aged 73 and 69 years respectively. Impaired visual acuity in both eyes was the chief complaint in both cases. Corrected visual acuity was 0.2 and light perception in one case and 0.01 and 0.4 in the other. All the affected 4 eyes showed signs of iritis and vitreous opacity. Funduscopy was possible in one eye only and showed diffuse chorioretinal atrophy. One case was diagnosed with malignant lymphoma by the presence of class V atypical lymphocytes by vitreous biopsy. In the other case,vitreous biopsy gave ambiguous results after topical corticosteroid proved futile. Intravitreal levels of cytokines showed a high possibility of malignant lymphoma by increased IL-10 and IL-10/IL-6 ratio. All the affected eyes received radiation therapy. Results：There has been no recurrence or metastasis to the central nervous system after follow-up for 9 and 13 months respectively. Final visual acuity was 0.3/hand motion in one case,and 1.2/1.2 in the other. Conclusion：Instillation of corticosteroid may decrease the positive rate in cytological diagnosis of malignant lymphoma. Intravitreal IL-10 and IL-6 levels may be of value as adjuvant diagnostic tool.
