神経線維腫症1型に合併した膀胱神経線維腫の1例

A case of bladder neurofibroma complicated with neurofibromatosis type 1

神経線維腫症1型（neurofibromatosis type 1：NF1）は常染色体優性遺伝であり，出生約3,000人に1人の割合で発症する最も頻度の高い遺伝疾患の一つである。皮膚のcafé-au-lait斑と神経線維腫を特徴とし，浸透率100％，表現型は様々である^1）。内臓の神経線維腫は10～20％程度でみられ，多くは腸管病変である。尿生殖器病変はまれではあるが，中では膀胱病変が多い^2，3）。今回我々は，NF1に合併した膀胱神経線維腫の1例を経験したため報告する。

A male in his 30s with a history of neurofibromatosis type 1（NF1）was incidentally found to be with a large mass infiltrating the posterior bladder wall on a gluteal MRI. T2-weighted images showed ring-like and septated patterns in the mass, which suggested a plexiform neurofibroma. A part of the tumor was transurethrally resected, and was histopathologically diagnosed as a plexiform neurofibroma. No malignant findings were seen. It was thought as a bladder involvement of NF1. Bladder neurofibromas with NF1 are rare. MR findings are specific and highly contribute to this type of diagnosis.
