膀胱発生の粘膜下腫瘤の多くは紡錘形細胞腫瘍である^1）。TUR-Bt後早期に術後瘢痕付近に生じ，画像検査上も病理診断上もpostoperative spindle cell nodule（PSCN）とgastrointestinal stromal tumor（GIST）との鑑別に難渋した紡錘形細胞腫瘍の1例について報告する。

A 70-year-old man was found to have a submucosal mass near the scar of TUR-Bt by cystoscopy. MRI showed a round submucosal mass in the posterior wall with heterogeneous high intensity on T2-weighted image. Histologically, there were spindle cells with negative for ALK, positive for c-kit and DOG1, and the findings suggested gastrointestinal stromal tumor（GIST）. Whereas, history of TUR-Bt, location and histological low cellularity strongly suggested postoperative spindle cell nodule（PSCN）.
