It is well-known that escalating an irradiation dose can improve treatment outcomes for prostate cancer patients based on the results obtained from previous randomized trials, but highly conformal irradiation techniques should be necessary to avoid severe morbidities after high-dose irradiation. Particle beams such as protons and carbon-ions have a common physical advantage in radiotherapy for deep-sheeted tumors including prostate cancer, and the particle beam therapy can be considered a reasonable alternative to photon-based radiotherapy such as intensity-modulated radiation therapy for prostate cancer at the present. To confirm the efficacy, multi-institutional prospective studies for proton therapy and carbon-ion therapy are now ongoing for the intermediate-risk and for the high-risk prostate cancer patients, respectively, in Japan.
