 We report a rare case of primary fibrosarcoma of the sacrum treated with carbon-ion radiotherapy. The patient was a 25-year-old woman who had a 1-year history of pain in the sacral region. Radiographic examination revealed a geographic osteolytic lesion having a trabeculated appearance in the lateral part of the sacrum. CT showed a purely osteolytic lesion with a thin sclerotic margin. On MRI the lesion was homogeneously low signal on both the T1-weighted and T2-weighted images, and it had extended into the spinal canal. Examination of a needle biopsy specimen yielded a pathological diagnosis of fibrosarcoma. The patient was treated with carbon-ion radiotherapy at a dose of 70.4 GyE. As of two years after treatment, the patient is pain-free, and there are no impairments of activities of daily living. Carbon-ion radiotherapy seems to be a promising alternative to surgery for the management not only patients of with unresectable tumors but of patients in whom tumor resection is predicted to result in severe loss of function.
