For early-stage centrally located lung cancer, the optimal dose fractionation for radiotherapy remains unclear. Stereotactic body radiotherapy(SBRT)carries a heightened risk of hemoptysis and bronchial stenosis due to the large doses administered. In contrast, moderate hypofractionated irradiation uses a lower fraction dose while maintaining a biologically effective dose of α/β=10―comparable to SBRT―and may provide a safer and effective alternative. Additionally, proton beam therapy theoretically allows stable dose delivery to tumors and fine adjustments to doses received by surrounding organs at risk, reducing the dose to mediastinal structures while minimizing lung exposure. Therefore, moderate hypofractionated proton beam therapy may be a safe and effective treatment option for centrally located lung cancer.
