A 50-year-old male presented with blurring of vision in his left eye. He had a foreign body sensation in the left eye while tilling the ground with a hoe. His left visual acuity was light perception. The intraocular pressure was 41 mmHg. The left eye showed traumatic cataract and hypopyon. B -mode echography and computerized tomography showed an intraocular foreign body which was removed by vitrectomy the same bay. As intraoperative finding, massive exudate was present beneath the internal limiting membrane (ILM). We removed the exudate after artificial detachment of ILM and added endolaser photocoagulation and gas tamponade. His visual acuity is 0.08 4 months after surgery without signs of intraocular inflammation. This case illustrates that prompt vitrectomy is indicated for endophthalmitis which manifest soon after penetrating ocular injury.
