A 37-year-old male was hit on his left face while falling down in an inebriated state. He visited us for left blepharoptosis 2 days later. His visual acuity was 1.0 right and 0.05 left. The left eye showed mydriasis,total ophthalmoplegia,and blepharoptosis. Computerized tomography showed a low-density rod-like shadow extending from the left orbit to the cavernous sinus. The finding led to the tentative diagnosis of orbital apex syndrome due to retained foreign body. A piece of wood,7 cm in length,was found and removed by surgery. His visual acuity recovered to 0.8 six months later. The fair visual outcome appeared to be due to early removal of foreign body and to sparing of the optic nerve from direct injury.
