Purpose:To report a case of mucosa-associated lymphoid tissue(MALT)lymphoma of bilateral lacrimal glands. Case and Findings:A 65-year-old male had swelling of right upper eyelid since 5 years before and of left upper eyelid since 1 year before. Bilateral lacrimal glands had turned into elastic hard tumorous mass. Magnetic resonance imaging(MRI)confirmed this finding. Biopsy of both lacrimal glands led to the diagnosis of MALT lymphoma. Gene arrangement analysis by Southern blotting showed positive rearrangement bands identical for both samples. This finding indicated that the MALT lymphoma in both lacrimal glands was derived from an identical cell clone. From additional clinical evidences,MALT lymphoma in the right lacrimal gland would have metastatized to the left. Systemic examinations showed metastasis to the neck and bone marrow,showing stage 4 MALT lymphoma. Conclusion:In the presence of bilateral MALT lymphoma,it is essential to perform gene rearrangement analysis in order to confirm monoclonality and to determine the pattern of clonality.
