間質性肺疾患合併Ⅰ期非小細胞肺癌に対する重粒子線治療成績：多施設前向きレジストリ解析（J-CROS-LUNG）

Clinical results of carbon-ion radiotherapy for stage Ⅰ non-small cell lung cancer with concomitant interstitial lung disease

重粒子線治療は放射線治療の一種で，重粒子（現在，臨床では炭素イオンが用いられている）を加速し，ターゲットに照射することで腫瘍制御を目指す治療法である。重粒子線治療は，国内では1994年に放射線医学総合研究所で臨床応用が開始された。2024年5月時点で国内7施設が稼働し，患者への治療が行われている。重粒子線治療は2000年代から先進医療の枠組みで実施されてきた。その治療適応や治療方針については，2014年に日本放射線腫瘍学会内に粒子線治療を統括する枠組みが設置され，統一治療方針の設定と全例データ登録を行う仕組みが作られた。各粒子線治療施設ではこの統一治療方針に則り，キャンサーボードによる適応判断が行われ，全例登録のうえで治療が行われている。

Particle therapy is particularly promising for early-stage non-small cell lung cancer complicated by interstitial lung disease（ILD）. In this article, we analyzed the results of a multicenter prospective registry study of carbon ion radiotherapy for early-stage lung cancer complicated by ILD. Thirty patients were included, and the 3-year overall survival, recurrence-free survival, disease-specific survival, and local control rates were 48.2％, 41.2％, 62.2％, and 88.1％, respectively. Adverse events were Grade 2 or higher and Grade 3 or higher in one patient（3.3％）, respectively, and no acute exacerbations were observed. Carbon ion radiotherapy can be safely administered to patients with ILD.
