慢性上顎洞炎に対する洗浄療法の有効な事は衆知の事であるがその有効例の大部分は先に発表1）したように3カ月以内の洗浄療法で治癒せしめえた。治療観察期間中は一切抗生物質などの薬物は用いず単なる生理的食塩水による洗浄のみを行なつているので，この治療効果は慢性上顎洞炎の治癒機転の解明に大きな糸口を示すものと考えられる。
上顎洞炎の治癒とは当然頻発していた洞内の細菌感染と病的分泌機能の終焉を意味している。洗浄液である生理食塩水そのものは感染微生物の増殖に直接的抑制作用は示さないので，この洗浄による感染防禦効果は宿主の防禦機構を介して発現されるものと考えられる。
In sinus secretion of chronic maxillary sinusitis, the antibacterial activity to Klebsiella and E. coli was determined by disk agar diffusion method in 34 patients. These activities were relatively stable to heat and resistant to freezing and thawing.
All 8 cases of chronic sinusitis in which the antibacterial activity was positive and bacteria in sinus secretion was negative were cured by the repeated sinus irrigation therapy with saline solution. On the other hand, in other 8 cases with negative antibacterial activity and positive bacteria, only 4 cases were cured.
The antibacterial activity in sinus secretion should have a important role in the healing process of sinusitis.
