荻野式上顎Osteoplastic operationおよびCaldwell-Luc法による上顎洞炎手術後の洞内状況観察について

OBSERVATION OF POSTOPERATIVE HEALING PROCESS WITHIN THE SINUS CAVITY

　慢性副鼻腔炎の手術方法に関してはDenker，Caldwell-Luc，Hayeck，Halle，和辻，西端その他先輩諸氏の各種各様な法式があり，またそれらの適切な組み合せもある。原法としては洞を洞として保存する事，あるいは骨，軟骨，脂肪，キールボーンなどによつて閉鎖する事の二法にしぼられるようである。がしかしいかなる方法が絶対的なものとの結論はいまだ得られておらず，各研究機関において種々検討が加えられているのが現状である。

　われわれは永い間Caldwell-Lucの方法によりこの手術を行なつてきたが，時に副鼻腔炎の再発，あるいは術後性頬部嚢腫の再手術に遭遇してみて，これらが，第一には手術野がせまいための病的粘膜の取り残し，あるいは上顎骨前壁の骨欠損部より侵入する肉芽のいたずらより発生する事を痛感し，上顎骨前壁の骨欠損部を被うべく横浜市立太学柏戸教授と機を同じくして上方に剥離した骨膜を下方に引つぱり，この骨欠損部を被い，歯齪に縫合する方法を試みた。しかしこの方法にしても，骨を軟部組織に変える事になるため，術者としては不安感がないとはいいえない。

　In spite of the numerous methods by which operations may be carried out in treatment of chronic sinusitis yet, there is none that could be considered as absolutely reliable.

　The present report is concerned with observation of the healing process in the maxillary sinus operated by modified Ogino method with a large intranasal opening on 150 patients. Daily observations of the sinus cavity were made possible by inserting fiberscope through the nasal opening. The advantage of the Ogino method is compared to that of the Caldwell-Luc.
