上顎洞の洗浄療法

ANTRAL IRRIGATION THERAPY

　副鼻腔特に上顎洞の病的粘膜の完全除去を目的とした上顎洞根治手術に対して最近その術後合併症あるいは症状の改善されない症例が多いため病的粘膜の正常への可逆性^1）2）を期待して粘膜を保存しかつ固有鼻腔と副鼻腔との交通を改善させる保存的治療が抗生物質，酵素製剤などの開発により脚光をあびつつある^3）。このうち洗浄療法は古くから行なわれその治療効果は注目に価するものであるが^4）7）最近は上顎洞に直接薬剤を投与しその局所効果を期待する研究が多く生理的食塩水のみによる洞内洗浄といつた効果に対して検討した論文は非常に少ない。

　今回われわれは昭和48年より昭和52年までに行なつた生理的食塩水のみによる上顎洞洗浄療法の効果を分折しその治療効果に及ぼす因子について考察したので報告する。

　The study of 90 cases (108 sinuses) with chronic empyema of the antrum furnished the interesting data with regard to the value of antral irrigations (once a week) with saline solution but without any antibiotics topically and also systemically. Of 108 sinuses, 33% of the cases were cured in one month, 66% in 3 months and 83% in more than 4 months of the therapy.

　Further analysis demonstrated that the cases with a single maxillary sinus involvement, negative culture for bacteria in sinus secretion were more curable than the cases of the multi-sinus involvement with positive culture.
