前部副鼻腔炎の手術法に就いて

　先人の長い間の努力にも拘らす慢性副鼻腔炎の治療は未だ完成されていない．私も一の試みを行い良好の結果を得たと信じて今年の総会に於いて発表した．その大要を茲に記載したいと存する．

　合併症のない慢性副鼻腔の手術は上顎洞をLuc-Caldwell法により，篩骨蜂窩を経顎的に開き，鼻内から補助的手術を加えると云うのが一般の趨勢であらう．後篩骨蜂窩は明視しながら徹底的に開放し得ようが，前篩骨蜂窩の開放は半腰盲目的に行わねばならず，不徹底であることをまぬかれぬ，第1表の示す樣に，前篩骨蜂窩は後篩骨蜂窩よりも強い炎症に罹つていることが多く之を徹底的に開放することが必要なのである．それにも拘らず手術が不徹底に終り，その結果中甲介の後半の浮腫，腫脹は去つても，前半のそれは減退せず，中鼻道前部は分泌物を見ることが多い．更に前額洞に病変があつて之を鼻内手術によつて治癒せしむることは今日猶相当に困難である．

　KITAMURA devises a method of operation whereby the ethmoids and the frontal sinus may be dealt with much ease when combined paranaaal sinus operation is called for. As the initial step of this procedure the maxillary sinus is disposed of as usual according to culdwell-Luc's method. But the pyriform aperture as well as the ridge are fully exposed extending the oral incision. The nasal mucous membrane is then separated up to the frontal angular process whence a is removed for gaining space with which to allow a full approach into the ethmoids and the frontal sinus.
