免疫学的立場から見た鼻副鼻腔炎の病態—特に副鼻腔病巣感染症についての検討

OBSERVATION OF PATHOLOGICAL STATE OF SINUSITIS FROM IMMUNOLOGICAL STANDPOINT

　鼻腔と副鼻腔とは，お互いに近接しているにもかかわらず，発生学的にも，また生理学的にも異なつている。すなわち，正常の副鼻腔粘膜は，血管，神経，リンパ組織，結合織など，いずれをとつても，鼻腔のそれに劣つている。しかしいつたん，感染が起こつた場合，簡単な構造を持つた副鼻腔粘膜でも，それに対する防御機構は十分備わつている。また感染が遷延した場合，今度は局所での防御の壁がくずれて，全身へ影響を及ぼす事になる。これが副鼻腔病巣感染症に通ずる考え方であるが，実際にはたとえば，自律神経系，内分泌系作用の変調など，複雑な因子が絡み合つている様である。本文では，副鼻腔病巣感染症の病態の一部を理解する目的で，最近経験した症例を中心に他疾患と比較し主として，1）鼻内分泌物，2）血清，3）組織の3点について，免疫学的立場から検討してみたい。

　Seven cases of focal infection in whom the sinus infection appeared to be the cause are reported. In 5 of these cases the bodily symptoms subsided after the operative cure of the nasal sinus infection, which was quite severe in each of them, was effected.

　By means of immunofluorescence method antibody formation in the sinus mucous membrane against γG and staphylococcus aureus was detected.

　Antibody against staphylococcus aureus was also found in the patient's blood.
