(24E-14) 眼内レンズ挿入術を行った388眼につき,個別A定数を求め,SRK/T式を検討した。前後面の曲率と厚さなど,眼内レンズのデザインが屈折力に応じて異なるため、A定数も同じく変化する。眼軸長に対しても個別A定数が変化し,これはレンズデザインによるA定数の変化よりも大きい。したがって,SRK/T式は広い幅の眼軸長には対応していない。また,SRK/T式は凸平レンズに対応した式であり,両凸レンズに対しては限界がある。このような理由から,術後の屈折が予測値の±0.5Dに収まったのは,症例の約50%であった。
From a review of 388 cases of cataract surgery with intraocular lens (IOL) implantation, I evaluated the individual A constants used in SRK/T formula. The individual A constant was actually a variable, because the anterior and posterior lens curvatures as well as the lens thickness changed according to different IOL powers and because the IOL was designed differently for each power. The individual A constant also varied with the axial length to a greater degree than the IOL design. The SRK/T formula therefore did not cover the whole ranges of axial lengths. Further, the SRK/T formula was intended for convex-plano IOL and not for biconvex one. Because of these reasons, the actual postoperative refraction was within ±0.5D from the predicted value in about 50% of cases in the present series.
