Abstract. We measured the axial length in 342 eyes undergoing cataract surgery with intraocular lens(IOL)implantation. We measured the axial length in each eye using three methods. A:It was measured optically just before IOL implantation with the anterior chamber filled with irrigating fluid. B:It was similarly measured with the anterior chamber filled with viscoelastic material. C:It was measured,before surgery,with conventional ultrasound. The error of refraction was expressed as(actual minus predicted refraction). It averaged-1.30±0.76 by method A,-1.20±0.73 by method B,and-1.87±0.92 by method C. The error of refraction by the three methods averaged-1.46±0.68. The value obtained by method C had the largest fluctuation(p<0.001). The averaged value of three methods had the least fluctuation and was the most accurate. The findings show the usefulness of using optically obtained axial length in SRK/T formula,and particularly using the averaged value obtained by the three methods.
