眼内レンズパワー計算の問題点

Problems in calculating the power of intraocular lens

　小切開白内障手術の普及とともに，質の高い術後視力が望めるようになってきた。それに伴い眼内レンズ度数計算に要求される精度が高くなってきている。当院で3か月以上経過観察できた62眼について，術後屈折値に与える要素について調べた。眼内レンズ計算式は修正したA定数でSRK／T式を用いた。全症例での予測精度は，予測値±1Dの範囲におさまった症例は87％，±0.5Dの範囲におさまった症例は56％であった。実際の術後屈折値と予測値との差は，倒乱視群でやや近視寄りに，直乱視群ではやや遠視寄りになる傾向を認めた。検査時の眼圧については，検査時の眼圧が低い症例は，高い症例に比べて，近視寄りになる傾向を認めた。

We reviewed 62 eyes who were followed up for 3 months after cataract surgery with intraocular lens implantation. We evaluated the factors that influence the postoperative refraction, comparing the predicted value by SRK/T formula with adjusted A-constant. The actual refraction was within ± 10 of the predicted value in 87% and within ±0.5 diopters in 56%. Eyes with against-the-rule astigmatism before surgery tended to be myopic and those with with-the-rule astigmatism tended to be hyperopia after surgery. Eyes with lower intraocular pressure tended to be myopic after surgery and vice versa.
