Investigation of individual A constant in single-piece intraocular lens

要約　目的：シングルピース眼内レンズ（以下，IOL）におけるパーソナルA定数を度数別，眼軸長別に算出し比較検討した報告。対象と方法：2011年1月～2013年2月に白内障手術を施行した146例252眼を対象とした。IOLはNS-60YG（NIDEK社）を使用。角膜曲率半径計測はTMS-5（TOMEY社），眼軸長測定はOA-1000（TOMEY社）のContactモード，度数計算式はSRK/T式を用いて予測屈折値を算出した。手術施行1か月後にオートレフラクトメーターARK-730（NIDEK社）を用いて他覚屈折値を測定した後，視力検査で自覚屈折値を算出，これを基にパーソナルA定数を度数別と眼軸長別に算出して個別A定数とし比較検討した。結果：IOL度数別の個別A定数は，17D以下ではパーソナルA定数より小さくなり，眼軸長別では25mm以上になるとパーソナルA定数よりも小さくなる傾向があった。結論：IOL個別A定数は，度数や眼軸長ごとに変化する。原因として，IOL度数ごとに変化するレンズ形状や度数計算式自体によるものが考えられた。

Abstract.　Purpose：To report we investigated personal A constant in single-piece intraocular lens（IOL）each IOL powers and axial length. Cases and Method：This study was made 252 eyes in 146 patients that received cataract surgery during from Jan. 2011 to Feb. 2013. The IOL we used was NS-60YG（NIDEK）. IOL power was calculated using SRK/T formula. A month later after surgery, we measured objective refraction by refractometer ARK-730（NIDEK）, and turned out subjective refraction by visual acuity examination. Based on this, we calculated A constant each IOL power and axial length and regard it as individual A constant. Results：Individual A constant of IOL power less than 17 diopter and axial length over 25 mm became smaller than personal A constant. Conclusion：Individual A constant was turned out to be changed each IOL power and axial length. These may be caused by IOL shape and calculated formula itself.
