シュレム管内壁擦過を付加したジヌソトミー併用トラベクロトミーとMMC併用ジヌソトミーの手術手技

Operation method of trabeculotomy combined with sinusotomy and sinusotomy with Mitomycin C added to scraping the inner wall of Schlemm's canal

（25M-9）　従来のジヌソトミー併用トラベクロトミー（LOT＋SIN）とマイトマイシンC併用ジヌソトミー（SIN＋MMC）手術にシュレム管内壁擦過を付加する術式を施行した。擦過はシュレム管内壁を高倍率の顕微鏡下で観察しながら25G針で丁寧に行う。内壁擦過の時期は，LOT＋SINではロトームの操作により内壁に損傷を加える可能性があるため，ロトミー施行後に行う。一方，ロトームを使用しないSIN＋MMCでは，シュレム管外壁切開切除後に擦過する。両術式とも内壁擦過時に穿孔した場合，レクトミーに変更せざるを得ない場合を考慮して，強膜弁打ち抜き施行前とする。シュレム管内壁擦過を付加することは，より良き緑内障手術への1つのアプローチとなりうるが，シュレム管外壁を十分に切開切除でき，ロトームの操作・内壁擦過を安全にするだけの熟練が必要であり，またその適応も考慮に入れなければならないと結論する。

Scraping the inner wall of Schlemm's canal is added to the operation of trabeculotomy combined with sinusotom (LOT +SIN) and sinusotomy with Mitomycin C (SIN + MMC) . Scraping is executed gently using the needle of 25 gaige by the microscope under high magnification. Because of a possibility of the trabeculotomy damaging the inner wall, the LOT + SIN carried this technique into effect after trabeculotomy is done. In the SIN +MMC, scraping is carried out after incision and resection the outer wall of Schlemm's canal. In both operations, this procedure is performed before punched out a screlar flap. We consider that this technique may be effective in an approach of better glaucoma surgery. However, it is necessary to exposed enough the Schlemm's canal, carried out safety trabeculotomy and reliable scraping the inner wall.
