（B5-2-3）　アレルギー性結膜疾患25症例（アレルギー性結膜炎9例，アトピー性角結膜炎7例．春季カタル9例）を対象に，少量の全血で測定可能なグラスファイバー法を用いた末梢血白血球ヒスタミン遊離試験（HRT）の有用性を検討した。患者についてRAST （radioallergosorbent test）法および結膜誘発試験（CPT）との比較を行った。HRTとRASTの判定一致率は72％から90％であった。HRTのCPTによる確定診断に対する判定一致率および陽性一致率（感度）は85％および78％であり，RASTはそれぞれ70％および55％であった。しかし感度はRASTがHRTよりも高かった。これらの結果から，RASTは原因抗原のスクリーニングに，HRTは原因抗原の確定に有用であることが示された。

We evaluated the diagnostic value of glass microfiber-based histamine release test (HRT) in 25 cases of allergic conjunctival diseases. This method allowed measurement using a small amount of the whole blood. The series comprised allergic conjunctivitis 9 cases, vernal keratoconjunctivitis 9 cases and atopic keratoconjunctivitis 7 cases. The cases were also evaluated by radioallergosorbent test (RAST) and conjunctival provocation test (CPT). The concordance of HRT with RAST ranged between 72% and 90%. The concordance and positive predictive value of HRT with CPT were 85% and 78% respectively and those with RAST were 70% and 55%. The sensitivity of RAST was higher than that of HRT. The findings show that RAST is useful in the screening of allergens and that HRT is useful in identifying the clinical allergens.
