A scanning laser ophthalmoscope (SLO) was used to evaluate early changes of epithelial herpes simplex keratitis in 50 eyes. All the eyes were examined for a period from a half day to two weeks (average 3 days) after onset of subjective symptoms. After staining the cornea by fluorescein, the cornea was observed by SLO using argon laser (488nm) . Observation was performed through a barrier filter of SLO with the anterior segment attachment ( +24D) . The findings were classified into dendritic keratitis 20 eyes and marginal keratitis 13 eyes and others 17 eyes. As a common feature, hypofluorescent dots in hypeffluorescent area, about 20-50 itm in size, were present in a microscopic dendritic arrange-ment or in a chain of small islands, which I propose to be called microdendritic keratitis. Presence of herpes simplex virus in the microdendritic area was confirmed in all 50 eyes using DNA in situ hybridization. Above findings show that it is possible to examine herpes simplex keratitis at the cell level in vivo using SLO.
