(B5-2-11) 走査型レーザー検眼鏡を用いて,上皮型単純ヘルペス角膜炎の初期病変を検索した。角膜をフルオレセインナトリウムで染色した後,アルゴンレーザーで励起しバリアフィルターを入れて観察する方法を用いた。ウイルスDNAの同定ができた50眼中,樹枝状角膜炎は20眼,辺縁部角膜炎は13眼,その他は17眼であった。これらの前眼部螢光所見で特徴的なことは,過螢光部の中に20〜50μmの低螢光点がみられたことである。さらにその配列は微細な枝状や列島状であり,微小樹枝状角膜炎ということができた。この検査方法は,本症の細胞レベルの解析に役立つだけでなく,今後の角膜疾患の研究に寄与することが期待される。
A scanning laser ophthalmoscope (SLO) was used to evaluate early changes of epithelial herpes simplex keratitis in 50 eyes. All the eyes were examined for a period from a half day to two weeks (average 3 days) after onset of subjective symptoms. After staining the cornea by fluorescein, the cornea was observed by SLO using argon laser (488nm) . Observation was performed through a barrier filter of SLO with the anterior segment attachment ( +24D) . The findings were classified into dendritic keratitis 20 eyes and marginal keratitis 13 eyes and others 17 eyes. As a common feature, hypofluorescent dots in hypeffluorescent area, about 20-50 itm in size, were present in a microscopic dendritic arrange-ment or in a chain of small islands, which I propose to be called microdendritic keratitis. Presence of herpes simplex virus in the microdendritic area was confirmed in all 50 eyes using DNA in situ hybridization. Above findings show that it is possible to examine herpes simplex keratitis at the cell level in vivo using SLO.
