　石灰化した内頸動脈が原因と考えられた偽緑内障の1例を報告した。症例は61歳女性で，近方視力低下を主訴として来院した。眼圧は両眼13mmHg，視神経乳頭には両眼ともに垂直C／D比が大きく篩状板部も透見可能な深い陥凹がみられ，視野検査では右眼に大きな弓状暗点，左眼に孤立暗点と鼻側段階が検出された。正常眼圧緑内障が考えられたが、除外診断の目的にて頭蓋内病変をCTおよびMRI検査にて検索したところ，右内頸動脈の海綿静脈洞部に著明な石灰化病変がみられた。脳血筥造影では，海綿静脈洞部から末端部（Cl〜C3 segment）にかけて，嚢状動脈瘤を伴うS字状に屈曲蛇行し拡張した内頸動脈が確認された。正常眼圧緑内障と診断される症例の中には，本症のように頭蓋内の血管性病変が視神経を圧迫する偽緑内障が存在することもあり，注意が必要と思われた。

A 61-year-old woman was referred to us with a recent history of decreasing near visual acuity in both eyes. Her corrected visual acuity was 20/20 each. Her intaocular pressure was 13 mmHg each. Funduscopy showed large glaucomatous disc cupping in both eyes. Automated perimetry showed superior nasal quadrantanopsia in the right eye. Computed tomography and magnetic resonance imaging showed a large calcified lesion in the C1-C3 segment of the right internal carotid artery. Angiography showed tortuosity and enlargement in the affected artery. This case illustrates that pseudoglaucoma may result as compressive neuropathy due to calcified internal carotid artery.
