　44歳女性の左眼に，原発閉塞隅角緑内障に対する線維柱帯切除術の直後に悪性緑内障が発症した。眼軸長が21mmで，前房がアトロピン点眼で形成されたことから，毛様体—水晶体ブロックと推測された。毛様体筋弛緩剤点眼による以後の治療13年間に，6回の発作があった。再発は，毛様体と水晶体の間隔が狭く房水が硝子体内に移勤し，上昇した硝子体圧により前進した硝子体が毛様体—硝子体ブロックを起こしたことによると推定された。最終的に，水晶体超音波乳化吸引術・後嚢切開・前硝子体切開術の同時手術で治癒した。

A 44-year-old female developed malignant glaucoma following trabeculectomy for primary angle closure glaucoma in the left eye. It appeared to be due to ciliolenticular block because of the short axial length of 21 mm and formation of anterior chamber by topical atropine. Attacks of malignant glaucoma recurred six times during the ensuing 13 years of treatment by cycloplegics. Recurrences appeared to have been caused by the shallow ciliolenticular space which led the aqueous to flow into the vitreous and induced anterior displacement of the anterior vitreous resulting in ciliovitreal block. Ultimate cure was achieved by simultaneous surgery by phacoemulsification, posterior capsulotomy and anterior vitrectomy.
