　67歳の男性が数か月間持続する左眼の変視症を主訴として来院した。矯正視力は0.3であった。左眼の眼底検査と光干渉断層計（OCT）にて，黄斑上膜に付着した硝子体の牽引による網膜剥離が黄斑部に確認された。耳側網膜の赤道部には孤立性の2乳頭径大の隆起した網膜血管腫がみられた。この網膜剥離による変視症を改善する目的で硝子体手術を予定していたが，初診日から40日後，矯正視力が0.7と向上したため再度眼底検査とOCT検査を行ったところ，後部硝子体剥離に伴い網膜皺襞，網膜血管の牽引は消失するとともに，黄斑部の網膜剥離も軽減し，また黄斑上膜の一部が硝子体に付着したまま剥離している所見が得られた。

A 67-year-old male presented with metamorphosia in his left eye since a few months before. His corrected visual acuity was 0.3 left. Funduscopy and examination by optical coherence tomography (OCT) showed an epiretinal membrane with macular detachment due to traction. A retinal angioma, 2 disc diameters across, was present in the temporal periphery. The traction retinal detachment showed, 40 days later, spontaneous resolution. The corrected visual acuity improved to 0.7. Funduscopy and OCT studies showed posterior vitreous detachment with peeling of the epiretinal membrane and reduced traction to the macula.
