偽緑内障の4症例

Four cases of pseudoglaucoma

緒　言

　緑内障の診断は眼圧，視神経乳頭，視野，隅角，トノグラフィー，各種誘発試験などの諸所見，検査の総合判断によつてなされるが必ずしも常に全症例が全ての所見を完備し，かつ全ての検査に陽性を示すとはかぎらない。そして一度緑内障との診断がなされると終生治療の対象となり，またその視機能の予後は患者の精神的重圧，苦痛となるから診断には慎重を期しても期しすぎることはない。

　ましてそのうちの一，二の所見または検査が緑内障を思わせたとしても安易に診断を下すことなく緑内障の疑いをもちつつ，反面それ以外の疾患も念頭におき，より一層多角的な検査をすすめるべきである。

Four cases of pseudoglaucoma are presented. The causes of optic atrophy with excavation proved to be, respectively, 1) aneurysm of caro-tid artery, 2) pressure atrophy of the optic nerve and the chiasm by sclerotic and ectatic carotids, 3) anterior ischemic optic neuropathy and 4) blunt head trauma affecting the optic nerve.The disc findings and the visual field defects in these cases closely simulated simple glaucoma.

The term of pseudoglaucoma is often confused with true glaucoma, but it must be discarded after the etiology of optic nerve changes is defi-nitely determined.
