（展示214）　48歳男性の右眼に白内障の嚢内摘出術が行われた。術後広範囲の網膜格子状変性が発見され，手術の7か月後にその部に円孔による網膜剥離が生じた。強膜の部分内陥術とジアテルミー凝固を行い，剥離のない格子状変性には光凝固を行った。光凝固で処理された部位から，以後6年の間に網膜剥離が3回生じた。最終的に冷凍凝固と強膜輪状締結術で治癒し，矯正視力0.9が得られた。嚢内摘出で硝子体脱出がなく，前部硝子体膜が残ったことが，網膜剥離の頻回再発にかかわらず，良好な視力が残存した理由と考えられた。

A 48-year-old male underwent intracapsular cataract extraction in his right eye. Extensive lattice degeneration was detected after surgery. Localized retinal detachment developed 7 months after surgery and was successfully treated by local scleral buckling with diathermy. Other sectors of lattice degeneration without detachment were treated by photocoagulation. Retinal detachment recurred three times from the photocoagulated areas during the ensuing 6 years. Final cure was achieved by scleral encircling with cryoapplication. Persistence of anterior vitreous membrane after uneventful cataract surgery appeared to have been good visual acuity in spite of recurrence of retinal detachment.
