白内障手術を施行した後部円錐水晶体の1例

Cataract surgery and histological findings of the posterior lens capsule in juvenile posterior lenticonus

　後部円錐水晶体を呈する4歳の男児において白内障手術の際に採取した水晶体嚢の病理所見を検討した。2歳時に羞明を主訴として当科を初診した際には右眼に軽度の後嚢下白内障を伴う後部円錐水晶体がみられた。経過観察中に白内障の進行により右眼視力が低下したので，白内障に対する手術を行った。術後にハードコンタクトレンズによる視力矯正と健眼遮蔽を行ったところ右眼矯正視力は術前0.3から術後4か月で0.8へ改善した。病理検査で後嚢円錐部に正常では存在しない上皮細胞層が観察され，本症の発症機序として円錐部の上皮細胞層の異常増殖が考えられた。

We performed cataract surgery in a patient with posterior lenticonus and examined histological-ly the posterior capsule of the lens. The patient was a 4-year-old boy, complaining of photophobia in his left eye. The patient was treated after surgery with occlusion of the fellow eye, and the visual acuity of the left eye improved to 0.8 versus 0.3 preoperatively. Histopathological examination showed abnormal multiple layers of epithelial cells in the area of the posterior conus. Abnormal proliferation of ectopic epithelial cells in the posterior capsule was the probable pathogenesis of the posterior lenticonus.
