Abstract Purpose:To report a case of posterior lenticonus who maintained good visual acuity without recourse to surgery.

Case:A girl aged 3 years 9 months was referred to us for poor vision in the right eye.

Findings and Clinical Course:Corrected visual acuity was 0.4 right and 1.0 left. The right eye showed conus-like protrusion of posterior lens capsule slightly posterior-inferior to the center. The posterior capsule showed slight opacity. State of refraction was +2.0D()cyl−2.5D 15° in the right eye and +0.5D in the left. The patient was treated by wearing of spectacles and occlusion in the better eye. Corrected visual acuity in the right eye improved to 0.9 three months later and to 1.0 nine months later. She has shown right visual acuity of 1.2 with stereopsis for 5 years until present.

Conclusion:The present case illustrates that full visual acuity may be maintained by wearing spectacles only in the presence of posterior lenticonus.
