鈍的眼外傷により後房眼内レンズが結膜下に脱出した1例

A case of traumatic dislocation of posterior chamber lens into the subconjunctival space

　偽水晶体眼が鈍的眼外傷を受け，術創が哆開して眼内レンズが球結膜下に脱出した1例を経1験した。症例は62歳女性で，手術はfornix baseの結膜弁を作成後，角膜輪部の後方約1mmの強膜に7mmの3面切開を行い，continuous curvilinear capsulorhexisを作った。水晶体を乳化吸引後，光学部直径6.5mmの3ピースレンズを嚢内固定し，強膜創は9-0ナイロン糸でshoelace縫合した。術後4か月後に扉の取手で左眼を強打した。9-0ナイロン糸が断裂して術創は哆開し，眼内レンズは上部球結膜下に脱出していた。手術創の狭い白内障手術後4か月を経過しても，強打すると術創が哆開して眼内レンズが眼外に脱出する危険があることが示された。

A 62-year-old female underwent phacoemulsification and posterior chamber lens implantation through a 7mm scleral incision. The surgical wound was sutured with 9-0 nylon thread. Four months later, she hit herself on the implanted eye with a door knob. The scleral wound ruptured along its entire length. The intraocular lens prolapsed, together with the lens capsule, through the wound into the subconjunctival space. Pseudophakic persons should be advised to avoid accidental trauma to the operated eye.
