眼内レンズ挿入術後に発症したPropionibacterium acnesによる遅発性眼内炎の1例

A case of delayed-onset pseudophakic endophthalmitis caused by Propionibacterium acnes

　眼内レンズ（IOL）挿入術後に発症したPropionibacterium acnesによる遅発性眼内炎の1例を報告した。症例は82歳の男性で超音波水晶体乳化吸引術およびIOL挿入術を施行し，術後経過は良好であった。術3か月後に右眼痛を自覚し，軽度の虹彩毛様体炎がみられたため，保存的に加療したが，前房内炎症は寛解と増悪を繰り返した。術8か月後に前房内炎症は増強し，水晶体嚢内には白色塊がみられた。このため遅発性眼内炎を疑い，IOLおよび水晶体嚢を摘出し硝子体切除術を施行，さらに，別のIOLを毛様溝に縫着した。手術時に摘出したIOLと水晶体嚢の細菌培養および組織学的検索によりPropionibacterium acnesが同定された。

A 82-year-old male underwent phacoemulsification and aspiration with posterior chamber lens implantation in the right eye. Ocular pain developed 3 months after uneventful postoperative course. Mild iridocyclitis was detected by slitlamp biomicroscopy. The condition resisted conservative treatments and, 8 months after surgery, white exudate was observed in the capsular bag. Under suspicion of delayed-onset endophthalmitis, we removed the intraocular lens and the lens capsule, performed vitrectomy and fixated another intraocular lens to the ciliary sulcus. Propionibacterium acnes was identified from the removed intraocular lens and the lens capsule by culture and histopathological studies.
