Abstract 文献概要

　走査レーザー検眼鏡に前眼部アタッチメントを取り付け，非接触型広角前置レンズを併用することにより，約75°の画角で眼底を観察する方法を考案した。この方法を用いて，正常眼10眼と糖尿病網膜症を中心とする各種疾患32眼を対象に，広角インドシアニングリーン眼底造影を行った。これにより，造影初期には後極から周辺に至る脈絡膜循環が一度に広い範囲で動的に観察できた。虹彩ルベオーシスのある症例（眼圧14〜46mmHg）では，動脈相が延長して脈絡膜動脈が明瞭に観察され，脈絡膜循環遅延が存在していた。脈絡膜血管腫など眼底の広範囲を占める病変では，正常部位との差が同じ時相で観察できた。またパノラマ法を用いることにより，各方向の渦静脈を越える周辺部までの脈絡膜血管構築が容易に観察できた。今回の方法を用いることにより，従来の方法では得られなかった広範囲の同時的な画像を得ることができ，脈絡膜全体での循環異常の解明に有用であった。

We developed a new method of wide-angle funduscopy by using a hand-held superfield non-contact lens as adjunct to scanning laser ophthalmoscope. The method allowed observation of the fundus at 75 degrees. We then performed indocyanine green angiography to observe the choroidal circulation in 32 diseased and 10 healthy eyes. In 4 eyes with iris rubeosis, the arterial phase was prolonged and choroidal arterioles were visible for several seconds, suggesting delayed choroidal circulation. By additional montage technique, we could observe the choroidal vessels peripheral to the site of vortex veins. The present method seemed to be of value in evaluating pathological changes in the choroidal circulation in extensive areas.
